In accordance with the relevant provisions for payment of fees and application of NCL, this authorization application will be processed only for local residents (including foreigners) in Taiwan(R.O.C.).

　

Please print out this form and personally signed after submitting your application, choose one of the way below to send:
1. Upload on website : Scan to PDF or take a picture in JPEG fromat, upload to “Authorization Contact” on Taiwan Memory website
2. Send by email to us : Send the scanned or photoed application form to our email (tmreader@ncl.edu.tw)
3. Send by post: Please mail to “no.20, Zhongshan S. Rd., Zhongzheng District, Taipei City 100201, Taiwan (R.O.C.) “ and indicate the recipient to “Taiwan Memory(Digital item authorization)”.
